Many of these decisions are based on emissions, fuel efficiency standards, and simple sales numbers. I know the mk8 Golf R is one of the few mass produced cars still coming to the US from a foreign importer in a manual configuration because the sales numbers supported it. The regulatory "cost" of changing something as substantial as a transmission means engines cannot have features like start stop and other emissions and fuel saving technologies. I am not saying it can't be done, but that a very small minority clamoring for something that ultimately hurts revenues isn't a good decision for staying in business.
