What can cost the oil cap cover to open up?

I went to put in windshield fluid and open the hood to find the engine oil cover off and some oil over the engine top cover and plastic around the cabin filter. I always make sure that it's securely fasten when I have to add a qtr of oil. What could of cost this to happen?

Does the cover feel loose even though its secured?

No; once I screw it it feels locked in place.

not screwing it in or leaving it open. i dont see how it can open if locked down tight.

Yep, that's the only way.
The cap will not just open by itself.

I added oil once and closed the cap but forgot to turn it.
The result was oil all over the underneath of the hood.
the only person to blame was myself!
