spark plug gap bridged, what is the cause?

07 328xi with ~75k miles, just got the car. Car had a slight miss on cold star-up. #5 plug, balance of plugs were worn but looked good. All were NGK PLZFR6A11S. Replaced with NGK LFR6AIX11

What would cause this? Dying coil?

I'd say that's a good guess. Carbon fouling due to improper spark (heat) generation.
