What is the difference between the index 9, and index 12 injectors? (Please forgive my ignorance)

The difference is that you can't mix index 12 injectors with index 9 or below and vice versa. so for example, if you had index 6 injectors and 3 go bad in bank 2, you can't put index 10 or above there because you have index 6. However, you could place index 9 injectors in bank 1.

Other alternative is that you will replace all 6 and update your fuel injection system to index 12.

Hope this helps.

So as far as flow rates and connections/ECU compatibility ANY index (9, 10, 11, 12, etc.) will work fine as a plug-n-play application, so long as all injectors being used are of the same index?

Yes, but you have to code them. They all have diff numbers on them to fine tune the spray pattern regardless of the index, so it's not just PNP.

Hmm.. well that's annoying, but not unexpected, is the coding something that can be done by an adventurous owner? Or does that require a dealership/specialty tool?

You can use INPA to code the injectors. Its fairly easy.

Take a look at @ http://www.e90post.com/forums/showthread.php?t=645844.

For my coding needs I went with a generic cable from ebay which set me back $15 and has already paid for itself. Needless to say that the cable with INPA can be used for various coding needs and diagnostics as well.

Good luck!
