Blown coil cylinder?

Hoping someone on the forum might be able to solve a mystery we ran across today while replacing spark plugs on an n52 engine. The plugs were changed fine, the coils are okay, there are no codes on the vehicle, and the vehicle is running fine. However we were baffled by the damage to the head on this engine, and could not explain what caused it. Anyone have ideas?
