I would double check all those items you listed. Also look at the hard vacuum lines. Are your wastegates closing?

Doesn't the car target lower boost when cold? I've never tried, but you shouldn't be going wot until the engine is over 80 degrees Celsius preferably 90.

It could be that you have a control line collapsing under vacuum when it gets warm too.
