Cylinder 1 injector failed twice in 2 months

I had swapped my injectors out to the series 12 injectors about 6 months ago. Then 2 months ago without warning my car threw the limp mode symbol with a very loud and rough idle. The codes I got were:
2E30- injection valve cylinder 1, input signal
30AC injection valve cylinder 1 input.

So this morning same thing again. Same codes. Anyone else have a similar issue or have any suggestions?

Is possible that you ECU is acting up, faulty engine harness or something mechanical in the head. Further diagnosis are required.

What year/DME? You wouldn't possibly be a 2007/8 with a MSD80 by chance?

Or maybe a MOSFETs issue?

Is your engine cover installed? Have you washed the engine?
