Thermostats are both changed, EGR valve has been cleaned, MAP sensor changed, EGR valve changed, MAF was measured. EGR was not that dirty but anyway who knows..I will remove the boost hose to see what happens

Actually it seems the problem is given by the torque converter lockup. At light throttle the lockup will be noticed and it causes the rpm to drop for 0,5 seconds. This causes the hesitation, what I find strange is in my 320d pre-lci I never had this issue and its an older generation of 6hp19.

I dont know if a FW update will help but a company in Austria is working on offering an app where you can flash your transmission ecu improving the switch times and response.

I'm probably being very thick here - but does that theory not apply only to Autos?

My manual 325d has the hesitation?

Yes this applies to auto but again its just a theory , so please share if you sort it out I am out if ideas what to try more
