Steering and Tachometer Problem E91 320d

I have an 08 (pre facelift) 320d E91 (manual).
We recently had a strange issue with the steering wheel feeling as if it is fighting back at you (similar to the feedback on a gaming steering wheel).
We took it to the garage and ended up getting the rack replaced. This fixed the problem for a couple of month but then the issue returned, this time with a steering dash warning light.
We then got the rack replaced again free of charge but the issue remains.
Any suggestions would be very welcome.
Since the most recent dash was fitted another issue has begun, see attached video, note during the video the car is travelling at constant speed in the same gear.
