NGK spark plug missing crush washer

I had a misfire in cylinder number six for quite a while but due to driving my car to work and back every third day to my fire department it wasn't critical that I found the issue.

I had a rough start since I installed these new plugs and sounded like my exhaust was blowing bubbles for the first 5 to 10 seconds and then it smoothed out, I figured this was due to a leaky valve cover which I had just replaced recently and still had the same issue so I started checking my plugs at first I didn't notice it, but when I was comparing two plugs together I noticed a crush washer was missing.
It was an easy fix go buy a new plug but I would have never thought that would've been the problem I guess it either fell off in the box prior to install or it never was on there because they seem to be to tight to fall off.
Anyway replaced the spark plug car runs great and strong, just wondering if anyone has ever come across something like this a simple fix but would be hard to fine.

I just bought some replacement BMW ZGR6STE2 spark plugs and the washers are copper or at least copper colored. I can see how not having a crush washer would make it impossible to torque it correctly. Good thing to watch for.
